Welcome to My Transition Plan! This is a resource for SPED instructors where learners will be creating a personal and realistic plan for after graduation. This activity can be completed in the school setting; however, if you are looking to pair this with CBI, then it is recommended you take your learners to a coffee shop! Learners will identify personal strengths, preferences, and interests, create a personal transition plan and identify supports needed in order to reach their goal.

This transition plan prompts learners to think about employment, housing, transportation, nutrition/exercise, enrichment programs, agencies/providers, education, leisure and personal goals. Learners will also identify strengths, preferences and interests!

i. Develop the topic with facts or details (EE.W.9-10.2b).

Resource most closely aligns with standard CCSS.SL.7.1b; however, fits in a range from middle school through adulthood.  Content is created to fit a modified standard.
